Alabama insurance complaint and appeal help

Alabama Department of Insurance. Use this page to find where to file an insurance complaint in Alabama.

External review

Insurers in this state use the federal external review process (HHS-administered or private accredited reviewers). Your final denial notice tells you which.

The department's page says it cannot help with plans that are not subject to Alabama insurance law, and notes that if a plan is self-funded your employer administers it, so you would contact your employer. (Source: Alabama DOI: File a complaint)

Before you file

  1. Appeal to the insurer first and keep copies; a complaint does not stop appeal or lawsuit deadlines.
  2. Check whether your plan is state-regulated: self-funded employer plans are usually outside state jurisdiction.
  3. Use the complaint summary letter to organize your answers, then use the department's own form.
