External review
This state operates an external review process that meets federal standards, per KFF. Your final denial notice names who handles it. For some plans, including self-funded employer plans, a federal process applies instead.
DIFI says that when an insurer denies a service or claim as not covered or not medically necessary, that is generally an appeal to the insurer, while other health insurance problems are complaints to DIFI. Its online system allows only one chance to attach documents.
Before you file
- Appeal to the insurer first and keep copies; a complaint does not stop appeal or lawsuit deadlines.
- Check whether your plan is state-regulated: self-funded employer plans are usually outside state jurisdiction.
- Use the complaint summary letter to organize your answers, then use the department's own form.
