Disclaimer: general information only
Insurance Appeal Helper is a free information site. Please read this before you rely on anything here.
Not legal, medical, insurance or financial advice
Everything on this site is general information compiled from public government, regulator and consumer-group sources for U.S. readers. It is not legal, medical, insurance or financial advice, and it is not advice about your particular situation or claim.
No professional relationship
Reading this site or using its letter builder or deadline estimator does not create an attorney-client, doctor-patient, claims-adjuster or any other professional relationship. The site is not an insurer, law firm, claims adjuster or medical provider, and it does not review your case or submit appeals for you.
Rules vary and change
Laws, deadlines and plan terms vary by state, type of plan and insurer, and they change. Your denial notice and your policy or plan documents control. Each page shows when it was last reviewed and links to its sources; information can still be out of date or may not fit your situation.
Medical information
Nothing here tells you what care you need or whether a treatment is medically necessary. Those are questions for you and your doctor or other treating clinician. Do not start, stop or change a treatment or medicine because of anything on this site. If you think you have a medical emergency, call 911 or your local emergency number.
Letter templates and deadline estimates
The sample appeal letters are templates and examples to adapt. They are not legal documents and not legal advice, and we cannot promise they fit your plan or your state. Review every line, remove anything that does not apply, and include only statements that are true for you. The deadline estimator gives rough estimates only; the date and instructions in your notice control.
No promise of results
No one can promise that an appeal, complaint or external review will succeed, that a claim will be paid, or that a deadline will be extended. Outcomes depend on your plan, your state, the facts and the decision-makers.
Who to ask
- Legal questions, lawsuit deadlines, or a deadline that has passed or is close: a licensed attorney in your state (many areas have legal aid or bar referral services).
- Complaints about an insurer or how a claim was handled: your state insurance department.
- Help understanding a denial or preparing an appeal: a patient advocate (some hospitals and nonprofits offer them) or a consumer assistance program.
- Medical questions, and any medical letter for your appeal: your doctor or treating clinician.
